<p> Before choosing a managed care plan make sure you have a list of services that you want it to provide. Sometimes a POS plan, because of its less-costly nature, does not offer all the services you might be expecting. Other than this you also need to ask a few questions from the company before you enroll into one of the managed care plans.<br/><br/>
<p>Some typical questions are: how many hospitals are covered under the plan you are about to purchas<span id="more-28"></span>e, what healthcare services are available, how much is the healthcare premium that you have pay, what are the deductibles and co-payments for non-network doctors and some other similar questions. This way you will be able to gain an insight into the operations of the health insurance you are planning to buy. If you are not satisfied with the answers or the plan does not seem to fulfill your needs, you can always consider an alternative.<br/><br/>
<p>You should also consider the number of doctors that are available under a managed care plan. If you plan to buy POS, this could be a very important step. If you need to see a specialist not listed on the network of doctors you may have to get a referral which is not always possible. So be sure that you are completely satisfied with the network to avoid heavy co-payments and other costs afterwards. However, there is always possibility of having the PCP convinced to refer an outside doctor. If that is the case, most of the charges of healthcare and other bills will be covered by the POS plan.<br/><br/>
<p>The managed care plans including POS are available as group policies, individual policies, medicare and medicaid. If you want to get a group policy you might have to get it through your job. Many group policy plans require that you stick with the plan at least for a year and then if you decide to change it you can do that freely. If the organization where you work does not offer group policies you may have to choose an individual policy. Individual policies are more expensive than group policies. Medicare is a Federal health insurance program and it covers the healthcare of people above the age of 65 and those will disabilities. Medicaid is designed for people with low income. Children and women who live alone can benefit from such a policy.<br/><br/>
<p>Also Some disadvantages of a POS plan like for non-network healthcare there are high co-payments and getting referrals for more specialists might not be an easy task. In addition for this there are deductibles in case of non network health care. However, this plan is overall less costly than PPO because the all this insurance companies covers most of your visits to physicians and specialists.</p>

<p> Italy&#8217;s national health care is rated second in the world by the WHO.  However, a closer look will show that there is trouble plaguing this system from a crippling bureaucracy, mismanagement, general disorganization, spiraling cost, and long waiting lines.</p>
<p>The Italian constitution was changed in 2001 so that the national government now sets the &#8220;essential levels of care&#8221; and regional governments still control their budgets and resources to<span id="more-22"></span> the local areas.</p>
<ul>
<li>Payroll taxes have a regressive structure starting at 10.6 percent of the first $30,000 of gross income and decreasing to 4.6 percent up to $100,000 gross income.  The remainder of the funding comes from both federal and regional general taxation, including income and value-added taxes.</li>
</ul>
<ul>
<li>In-patient and primary care are free at the point of treatment. However, co-payments are required for diagnostic procedures, specialist, and Rx drugs. The copays run around 30 percent of the services rendered. The elderly, pregnant women and children are exempt from the copays&#8211;which is nearly 40 percent of the population.</li>
</ul>
<ul>
<li>Italians have limited choice of physicians.  They must register with a general practitioner within their LHA.  They may choose any GP in the LHA, but may not go outside it except for emergency care.  A referral from a GP is required for diagnostic services, hospitalization, and treatment by a specialist.</li>
</ul>
<ul>
<li>Most physicians are reimbursed on a capitated basis, which is based on the number of patients served over a given time period rather than the services actually provided. Some hospital physicians receive a monthly salary.</li>
</ul>
<ul>
<li>Private health insurance is available in Italy but is not widespread. About 10 percent of Italians have private insurance, and the low percentage is due that one cannot opt out of the national care system.</li>
</ul>
<ul>
<li>Waiting periods on average for medical care:  70 days for a mammogram, 74 days for endoscopies, and 23 days for a sonogram.  This is due to shortage of modern medical technology.</li>
</ul>
<ul>
<li>The US has two times as many MRI units per million people and 25 percent more CT scanners.</li>
</ul>
<ul>
<li>Introduction of many of the newest and most innovative Rx drugs have been blocked by the Italian government to control Rx cost.</li>
</ul>
<ul>
<li>Conditions in public hospitals are considered substandard, particularly in the south.</li>
</ul>
<ul>
<li>Dissatisfaction with the Italian health care system is extremely high, by some measures the highest in Europe.  Fifty-five percent of Italians believe that it should be easier for patients to spend their own money on health care.</li>

<p> Funny thing pain, if you&#8217;ve never had a severe pain then the suggestion of taking simple analgesia and resting the affected area all seems quite reasonable. I was reminded of this when I read recently of a doctor&#8217;s advice to someone who was suffering from sciatica. Having personally experienced sciatica, it&#8217;s a condition I would not recommend to anyone who wishes to walk, sit, laugh, sleep, or to just simply pull up your trousers. It&#8217;s a bit like<span id="more-18"></span> a dentist drilling your teeth without an anaesthetic, but it affects your whole leg. In other words the pain is consuming, exhausting and without respite. Clinical studies do show that in the majority of cases the pain will eventually subside and surgery may not be necessary, but in the meantime the patient has to deal with the pain or deal with the medication required to dull the pain. Remember, pain-killers are not selective to the area affected. They affect the whole of the nervous system and elsewhere so there may be significant side-effects from these medications.</p>
<p>Dealing with severe pain can be a complex issue, but I suggest that you have to treat this sort of pain fairly aggressively as acute severe pain is relatively easier to treat than chronic severe pain. In the early stages of an injury or insult to an area of the body, most of the pathological processes are happening at the site of the injury or insult. Throughout time the brain begins to modulate this pain and so no only do you have the injured area to deal with, but you also have complex neural pathways within the brain to deal with as well. This often means a far more complex management plan and a far more protracted recovery time. Specialists are very skilled at dealing with these issues but they do rely heavily on the stories their patients give them. That means being honest in answering their questions and not being heroic with a grin and bear it grimace! Often the use of a pain scale is helpful with zero being no pain at all and a 10 being the worse pain you have ever experienced.</p>
<p>Another health issue we commonly down play is influenza. Over the years I have frequently heard people say that they would not have the flu vaccine because either they never get the flu or that they had it last week for a couple of days and then it was all over! Influenza is a serious debilitating disease that will usually last from 10 days to two weeks and often leave you flat on your back exhausted. It&#8217;s not a happy 10 days either as patients do not have the energy to read a magazine or even watch a DVD. You will literally feel ancient with every movement being a real challenge and that doesn&#8217;t include the aching all over or the fevers and sleepless nights. The influenza virus is also extremely contagious and most people are unaware that if you spread it to someone who is more frail than yourself that you may actually be putting their life at risk.</p>
<p>With the &#8216;flu the big challenge is to vaccinate as many people in the community as possible, including children, those employed and unemployed, the elderly and the infirm, to reduce the chance of an epidemic occurring. Recent research has also showed that vaccinating pregnant women in the last trimester of their pregnancy will help protect their new born infants born during the &#8216;flu season.</p>
<p>Medicine has evolved over the last 40 years, but the change has been fairly slow with doctors by nature being very cautious and conservative people. But we can&#8217;t leave the doctors to take all the initiatives. As patients we need to be good listeners in our approach to health by heeding all the great health messages that keep being given to us about vaccinations, smoking, alcohol, exercise and healthy eating. We also need to be good communicators and tell our doctors how we are feeling with conditions such as pain. If the team treating you doesn&#8217;t have the best information then it may be that you will not end up getting the best treatment!</p>

<p> My son is a micro-biology major at the University of Maine. I raised that young man on an essentially vegetarian diet, and I was careful to avoid foods with some of the more colorful chemical additives in them, such as MSG and artificial sweeteners. My reasoning was that I could not afford to buy such foods.</p>
<p>Still, on his last visit home, he bought a McDonald&#8217;s hamburger, saying , &#8220;Yumm, food. It&#8217;s all organic. If it were inorganic it would<span id="more-21"></span> be a mineral, or a chemical, or something else.&#8221; He was right, of course. Still, we have the understanding that good food does not contain unnecessary chemical additives, which could be harmful to our health.</p>
<p>I looked at the title of one of our popular magazines &#8211; Natural Health &#8211; and one of my first thoughts was, yes, health is natural. The body metabolism seeks balance, a state of equilibrium and well being, and that is about as natural as one can get.</p>
<p>True, our drug companies, along with the food industry are far more interested in turning a profit than they are in ensuring the health of this nation&#8217;s consumers, and the controls that should be in place to protect us have been weakened to the point that they are no longer credible. So many news stories of medications released for public use, which later turned out to be extremely harmful and had to be taken off the market, have been circulating, that we are becoming very suspicious of what the AMA as a profession has to tell us. So, many of us are now looking for ways to maintain and protect our health that are not expensive, as deep down we all believe that the means to good health are our birthright, and that they should not impoverish us.</p>
<p>Unfortunately, what falls under the heading &#8211; Natural Health &#8211; is too often dominated by the products of large corporations that sooner or later espouse the same ethic that guides every other corporation in this country&#8211;do everything to keep the value of the stocks high. Even the companies that are not publicly trading their stocks are competing with companies that do this. And as we know, any company that publicly trades stocks owes a higher allegiance to its stockholders than it does to its customers.</p>
<p>Herbal medicine was essentially free, or if not free, the women who practiced it would not accept money for the help they gave. Of course, separating the food one eats, and the house one lives in, from money is an almost impossible feat. Perhaps in the pre-industrial world, when more people used less money and housing was not the major expense that it is now, a woman could support herself that way. Still, health care should not be a major expense, and herbs and simples are more effective than many people would suspect.</p>

<p> Hopefully, the only health care that any person will need is preventative health care. Each person should start with preventative health care so they will never need anything to cure their health problems. Health should start with good nutrition and exercise. There are many professionals who provide great information on proper nutrition that will keep people healthy so they will never need any drastic measures to cure a dreaded disease. There are<span id="more-20"></span> also some great experts who can advise people on the exercise that will keep them healthy and happy.<br/><br/>
<p>Preventative care also includes consultation with medical professionals on a regular basis to make sure that there is nothing going wrong in the body and mind. People are often advised to check with their doctor on a yearly basis for a basic set of tests to see if there are any problems. People will often have some blood tests which will provide information on the health of that person. There are other tests that do not need to be completed every year, but these tests should be done periodically. After the age of forty, women usually have a mammogram on a regular basis according to their doctor&#8217;s advice.<br/><br/>
<p>Health Care Cures A Person&#8217;s Ailments<br/><br/>
<p>After a person completes the recommended tests, a person will meet with their physician to hear about their health. The physician will know if there is anything wrong, and then he or she will prescribe treatment or congratulate a healthy person. The physician could recommend drug therapy or surgery if there are any problems. The doctor might remind the patient about good nutrition and exercise if the person has not been conscientious about nutrition and exercise. After consultation with the doctor, he or she should provide treatment for a brief period of time or treatment that will last an extended period of time.<br/><br/>
<p>Once the health provider makes some recommendations, each person should follow the recommendations to ensure their health is effective. Those who follow the directions will probably have some success in reversing the unacceptable conditions. Some may take longer than others to recover from their ailments, but most people will find treatment for their ailments. There are some conditions that may take a year or more of treatment while other people will take some medicine all of their lives to treat all of their ailments. Each person working with the people dedicated to health care should find great success.</p>
